Provides spiritual support or visits at the time of death as assessed by patient/family need. Will support the admission / consent process in collaboration with the inter-disciplinary team in the explanation of hospice benefits to the patient and families at leadership request. Supports bereavement care plan to surviving family members for up to 13 months after patient dies; including phone support, in home visitations, bereavement support groups and memorial. Chaplains assist with funeral services for deceased patients when requested and as scheduling allows.
